You can send it to us via regular mail, If it will arrive in time for your appointment, or bring it with you or fax it to us at (845)-226-1464. 

If you wish to inquire whether we are participating with your particular insurance or you cannot find our name in your provider manual, please call our office with the telephone number of your insurance. We will inquire further.
